Outreach team
The team is a mixture of specialist health professionals and trained support workers, carrying out daily outreach sessions.
Outreach visits vary between early morning and late evenings, to help ensure as many reported rough sleepers are visited and support provided.
Once a person is identified as rough sleeping the necessary steps are taken to arrange for a Housing Assessment to be carried out by a member of staff from the Changing Futures Team, who search for any suitable accommodation provision available.
How our Outreach Nurse helps those we support
Sophie provides support to people who are rough sleeping within the city of Hull, by bringing the necessary healthcare directly to them.
Support includes -
- wounds care
- blood borne virus screening, including Hep B, Hep C and HIV
- Hepatitus B vaccinations
- offering harm reduction advice
Sophie also provides support with substance treatment, and helps to connect individuals to other health services, when needed.
