Making Every Adult Matter (MEAM) began in Hull in 2018. The service was introduced to support individuals facing multiple unmet needs whose circumstances were shaped by long-term experiences of -
- poverty
- deprivation
- trauma
- abuse
- neglect
The service now sits under Changing Futures. Our aim is to design and deliver better coordinated services for individuals. The work starts with partnership, co-production and vision. We want to get together the right providers from different sectors, commissioners and people with lived experience to agree a shared understanding of the problem and a vision for the whole area.
We work to make sure Hull has a strong partnership approach to multiple unmet needs in place and is taking practical steps to transform services and local systems.
The team consists of -
- MEAM Manager and Senior Case Workers
- MEAM Navigators
- DWP worker
- senior drug and alcohol recovery worker
- probation service officers
- domestic abuse workers
- adult social care manager
- social worker
- occupational therapist
- neuro navigator
We work alongside the Rough Sleeper Initiative Team and the Adult Social Care Team under Changing Futures.
